48-HOUR PARTICIPATION DECLARATION
A final check before we meet outdoors.
This declaration checks whether anything has changed since your original questionnaire and confirms that you have received the practical and safety information for your retreat. A change does not automatically mean you cannot attend—please answer honestly so we can discuss what is safe and appropriate.
WHEN TO COMPLETE Within the 48 hours before your retreat, and before travelling to the meeting point.
This form is not a medical assessment or a substitute for advice from a qualified healthcare professional. If you are unsure whether the planned activity is suitable for you, please seek appropriate medical advice before attending.
